Most men over 45 are taking either nothing or too much. The nothing group assumes supplements are marketing. The too much group has eleven bottles on a shelf, most bought after a single article, none reviewed since. Both are making the same mistake, which is treating the question as a shopping problem rather than a sequencing problem.
This is the order that actually makes sense for a man in his mid-forties or beyond, what the evidence supports at each step, and where the honest limits are. No supplement on this page replaces a diet, a blood test, or a doctor. What follows assumes you have those or are getting them.
Start with the four things that are not supplements
Every credible answer to this question begins the same way, and skipping it is why most supplement routines disappoint.
Sleep. Short sleep affects appetite regulation, glucose handling, mood, and recovery more than any capsule will. If you are running on six hours, fixing that changes more than a cabinet full of bottles.
Protein and resistance training. Muscle mass declines from roughly the fourth decade onward. Resistance training two or three times a week plus adequate protein is the single most effective intervention available to a man over 45, and it is free. Most men in this bracket are under-eating protein rather than over-eating it.
Bloodwork. You cannot supplement intelligently without knowing your baseline. Vitamin D, ferritin, B12, lipids, HbA1c, and for men over 50 a PSA conversation. We covered which tests to ask for in a separate piece on blood tests men over 45 should ask for.
Alcohol. It affects sleep architecture, liver function, and recovery. Reducing it outperforms most supplement decisions and it is the one most men would rather not hear.
If those four are not in reasonable order, supplements are a rounding error. If they are, the rest of this page is worth your time.
Tier one: correct the deficiencies you probably have
This tier is not exciting and it is where the real return is. You are correcting a shortfall rather than adding an effect, and correcting a shortfall is the only situation where a supplement reliably does something you can feel.
Vitamin D3
Insufficiency is common, particularly at northern latitudes, in men who work indoors, and in winter. It matters for bone metabolism, immune function, and muscle function. This is one of the few cases where testing is cheap, widely available, and directly actionable: measure, correct, re-measure.
Typical maintenance sits in the 1,000 to 2,000 IU range, but a genuine deficiency needs a higher corrective dose and that is a conversation with your doctor rather than a guess. Vitamin D is fat-soluble and can accumulate, which is the reason the "more is better" instinct is wrong here specifically.
Take D3 rather than D2, and take it with a meal containing fat. Absorption differs meaningfully between a capsule swallowed with coffee and the same capsule swallowed with breakfast.
Vitamin K2
K2 works alongside D3 in calcium handling, activating the proteins that direct calcium into bone rather than leaving it in soft tissue. The two are usually discussed together for that reason, and we go deeper into the pairing in vitamin K2 and D3 for men.
The form matters. MK-7 has a substantially longer half-life than MK-4, which means once-daily dosing works. Most products that list "vitamin K" without specifying are using K1, which is a different thing with a different job.
One caution that matters: if you take warfarin, vitamin K directly interacts with it. Do not add K2 without speaking to whoever manages your anticoagulation.
Magnesium
Involved in hundreds of enzymatic reactions, including muscle function and sleep quality. Intakes below recommended levels are common. Glycinate and citrate are better tolerated than oxide, which is poorly absorbed and mostly acts as a laxative, and which is what most cheap magnesium products contain.
Check the elemental magnesium figure rather than the compound weight. "Magnesium citrate 500mg" might be 80mg of actual magnesium. This is the same disclosure problem that runs through the whole category.
Omega-3
If you eat oily fish two or three times a week you likely do not need a capsule. If you do not, EPA and DHA are worth considering, and this is one of the better-studied areas in the whole supplement category.
Look at the actual EPA and DHA numbers on the back rather than the "fish oil 1000mg" claim on the front, which describes the weight of the oil rather than the active fraction. A 1000mg capsule commonly contains 180mg EPA and 120mg DHA, so hitting a meaningful daily intake can take three or four capsules of a cheap product and one of a concentrated one. Work out the cost per gram of EPA plus DHA and the cheap option often stops being cheap.
B12
Absorption declines with age and is reduced further by metformin and long-term proton pump inhibitors. If you take either, this belongs on your test list rather than your shopping list, because supplementing B12 without measuring it can mask a deficiency that has another cause worth finding.
A quicker way to judge a bottle

Tier two: the age-specific additions
Once the deficiency tier is handled, these are the ones with a reasonable rationale for men specifically in this age bracket.
Creatine monohydrate
The most studied sports supplement in existence, and increasingly interesting for older men rather than only for athletes. It supports strength and lean mass alongside resistance training. Three to five grams daily of plain monohydrate. The fancier forms cost more and have not outperformed it.
Two practical notes. There is no need to load; taking three to five grams daily simply saturates more slowly and arrives at the same place. And it is worth saying plainly that creatine only does much if you are training. Taken alongside a sedentary routine it is not doing the thing the research describes.
Circulation support
Blood flow underpins a great deal of what men notice changing after 45, from energy through the afternoon to drive and stamina. The nitric oxide pathway becomes less efficient with age, and this is the layer that sits underneath a lot of the symptoms men attribute to other causes. The botanical with the most research behind it on that pathway is icariin, from epimedium. The critical detail is standardisation: "epimedium extract, 1000mg" tells you the weight of the powder, not how much icariin is in it, and that can vary enormously between suppliers. A label that says "standardised to 100mg icariins" is telling you something. A label that does not is telling you something too.

Prostate support
Urinary symptoms become more common with age and are worth taking seriously rather than normalising. Saw palmetto is the most commonly used botanical here and the evidence is genuinely mixed, with some good trials showing no separation from placebo. It is reasonable to try, unreasonable to expect it to resolve significant symptoms, and important not to use it as a reason to delay getting checked. Bone and cardiovascular support
Men lose bone density too, later than women but from a similar mechanism, and it is almost never discussed until something breaks. Calcium intake, vitamin D, K2 and resistance training are the levers. See bone density in men over 50, and Frame & Engine covers this pairing.
Tier three: mostly skip these
Not because they are harmful, but because the return does not justify the cost or the evidence does not support the marketing.
Testosterone boosters. The category is largely built on a claim that is difficult to substantiate. If you suspect low testosterone, the answer is a blood test and a doctor, not a bottle sold on the internet. The honest position is that no over-the-counter botanical has been shown to move testosterone in a way that reliably changes how a man feels.
Tribulus terrestris. Included in a great many "T-booster" formulas on the strength of animal work that did not replicate in men. It is the clearest example in the category of an ingredient whose reputation outran its evidence.
Zinc, unless you are low. Genuinely necessary, genuinely involved in hormone production, and genuinely unhelpful to supplement if your intake is already adequate. Long-term high-dose zinc also interferes with copper absorption, which is a real downside for something taken speculatively.
Arginine on its own. The nitric oxide logic is sound, but the doses used in research are in the region of three to six grams. Most blends contain a small fraction of that, which makes the ingredient list accurate and the effect unlikely. Citrulline has better oral bioavailability if you want to pursue this route.
CoQ10, for most men. Genuinely useful for men on statins, since statins lower circulating CoQ10 as a direct consequence of how they work. Much weaker case for a healthy man expecting an energy improvement. The detail is in CoQ10 benefits for men.
Multivitamins. Not harmful, and a reasonable insurance policy if your diet is genuinely erratic. But they typically deliver small amounts of many things rather than a useful amount of anything, and they are often used as a substitute for identifying what you are actually short of.
Anything with a proprietary blend. That phrase is legal permission not to tell you how much of anything is inside. There is no version of that which benefits you.
When to take what
Timing is a small effect compared with whether you take something consistently, but a few of these are worth getting right because they change absorption rather than convenience.
With a meal containing fat: vitamin D3, vitamin K2, omega-3. These are fat-soluble and taking them on an empty stomach measurably reduces how much you absorb. Omega-3 with food also cuts the reflux and fishy aftertaste that makes most men quit.
Evening: magnesium, if you are using it partly for sleep quality.
Any time, consistently: creatine. Saturation is what matters, not timing.
Away from coffee and tea: iron, if you have been told to take it. Polyphenols in both substantially reduce absorption. This does not apply to most of the list.
The wider point is that a routine you follow four days a week is worse than a smaller routine you follow every day. Build for the version of yourself who is tired and in a hurry.
What not to combine without asking
Supplements are not inert, and the interactions that matter are mostly with prescriptions rather than with each other.
Anticoagulants. Warfarin with vitamin K is the obvious one. Omega-3 at high doses, and several botanicals, also affect clotting. If you are on any blood thinner, every addition goes past your pharmacist first.
Blood pressure medication. Anything acting on blood vessels can stack with antihypertensives. Worth a conversation rather than an experiment.
Nitrates. If you take nitrates for angina, do not add supplements marketed for blood flow without medical advice. This is a genuine interaction, not a disclaimer.
Diabetes medication. Several botanicals affect glucose handling, which matters when a medication is already doing that.
Your pharmacist is free to ask, does not need an appointment, and will know the interactions better than any article. Take a photo of the labels and go in.
How to actually judge a label
Five checks, about ninety seconds per bottle.
Is every amount printed? No proprietary blends. A company that put a meaningful dose in has no reason to hide it.
Is the extract standardised? And to what compound, at what amount.
Does the dose match the research? Many products list an impressive ingredient at a fraction of the amount that was actually studied.
How many servings are really in the container? Sixty capsules at two per day is thirty days, not sixty. Work out the real cost per day.
Where was it made? An FDA-registered, GMP-certified facility is the baseline rather than a distinction.
How long to give it, and how to judge
This is where most routines fail, because men judge on the wrong timeframe with the wrong evidence.

A reasonable starting routine
If you want the short version for a generally healthy man over 45 who trains and has had recent bloodwork:
Vitamin D3 with K2, magnesium in the evening, omega-3 if you do not eat much fish, creatine daily if you are lifting. Add circulation support if drive and afternoon energy are what you actually want to address. Add prostate support if urinary symptoms have started. Revisit in three months against how you feel and your next set of bloods, not against how you felt in the first fortnight.
That is four to six items, not eleven. The discipline is in what you leave out.
